20 Tips to Consider When A Loved One Is Referred to a Physical Therapy Rehabilitation Center

IRFs are free-standing rehabilitation hospitals and rehabilitation units in acute care hospitals. Patients who are admitted must be able to tolerate three hours of intense rehabilitation services per day. Common diagnoses for patients who require intensive inpatient acute rehabilitation therapy include: Stroke, brain injury, spinal cord dysfunction, heart surgery, amputation, neuromuscular conditions, arthritic conditions, joint replacement, and other conditions. Their primary focus is to provide rehabilitative services to assist patients in becoming as independent as possible in their activities of daily living so that they may return home and re-enter the community. Below are twenty tips to consider when your parent/loved one is sent for inpatient physical therapy rehabilitation.

MEDICATIONS:

1. Provide a list of all current medications to the rehabilitation center so that no dosages are missed.

2. Do not bring in or give the patient any unauthorized medications/supplements from home without the knowledge of the treatment team.

COMMUNICATIONS:

3. Inform the patient’s primary care physician (PCP) that the person has been admitted to the rehab center to allow for coordination of care.

4. Provide the contact information including the name and phones numbers of the current providers including the PCP, Cardiologist, Podiatrist etc.

5. Make sure to designate one person as the point of contact for coordination of care and treatment planning.

6. List at least two additional persons as emergency contacts with both home/work and mobile telephone numbers.

7. Visit regularly and consider alternating family members to prevent possible caregiver burnout.

8. Speak to the Attending/Treating Physician regarding the outlined plan of care and do not hesitate to ask questions regarding course of care.

9. Bring a copy of any prepared advanced directives for placement in the patient’s medical chart. If no advance directives were completed, consider completing one at the rehab center.

10. Discuss any physical, mental or emotional changes you notice immediately with the medical staff.

Sleep and Bullied Kids

One thing that picked-on kids and adults have in common is their inability to relax and sleep. Unless the sleep is used as a type of escapism, deep rest is often difficult for the bullied.

I know as a kid, I had constant nightmares from listening to my mother and step-dad argue. I got to the point of being afraid to sleep. Later, I liked the fatigue as it shut out the pain and monotony of high school. It got worse when I worked late or early in the mornings. My school grades suffered and so did my health.

(Of course, when you are hyper-vigilant of drunken step-dads and high school bullies, you have difficulty relaxing.)

Despite how much I ate, I could not seem to gain much muscle. It was not until years later, when I became more independent and confident that I was able to rest and sleep deeper and gain more muscle weight. I could concentrate better as well. So, later I attended university and would actually remember what I was supposed to.

Rest is often ignored by those in a hurry to lose weight, gain muscle, get through school or learn to defend themselves. That is often because the adrenaline glands are stuck on high. Trust me on this one.

To support this theory, I read about a study in Montreal, Canada, in 1958, where school children were on meal programs and generally ate about the same amount of food. Fairly consistently, the children from high-stress families almost always were under-weight and tired.

Well, this was a relief as I realized that I was not a total moron. My mind could not process information while stuck in the survival mode most of the time.

So, what does this means to you, my friends, who might be struggling with under or over-weight (too much cortisol from stress)?

It means that to make greater muscle gains, fat burning and improved concentration, you must include rest and sleep in your routine. If you have topped up your school course load and are holding down two jobs, then it is time for a serious rescheduling as the lack of sleep can ruin your school marks, ability to earn a living and your relationships.

I used to work 12 to 18 hour days all summer and then return home to school and/or work and be tired and restless for about three weeks while my mind and body tried to recover from the hectic pace of training with the army reserves. Sleep is nature’s loan shark. It will collect that recovery period eventually.

Survey Shows Rise in Depression, Suicidal Thoughts in US Teens

A nationwide survey of teenagers in the United States has revealed a rise in fear and despair among them, with one in seven admitting to the misuse of prescription drugs. Since 2007, there has been a rise in the number of teenagers reporting feelings of dejection and despair. Suicidal tendency and absenteeism in school have gone up due to the fear of bullying and violence. The trend has been noteworthy in the lesbian, gay, bisexual and transgender (LGBT) community in high schools.

Countrywide, one out of five students reported facing bullying at school, one in 10 females and one in 28 male students reported having been subjected to forced sexual activity. Dr. Jonathan Mermin of the Centers for Disease Control and Prevention (CDC), which conducted the Youth Risk Behavior Survey, said that the life of an adolescent can be challenging. However, a huge number of students admitting to persistent feeling of hopelessness and 17 percent contemplating suicide tell the sad state of affairs.

In 2007, 28 percent of teenagers reported to have suicidal feelings, which rose to 31 percent in 2017. Similarly, 14 percent of teens made suicide plans in 2017 as against 11 percent in 2007. The survey, conducted every two years, involved 15,000 high school students across 39 states. It asks questions pertaining to a wide range of activities and attitudes.

There were some positive observations as well. Compared to a decade ago, fewer teens reported indulging in sexual activity, consuming alcohol or taking drugs like marijuana, heroin, and cocaine. Since the question related to prescription opioid was asked for the first time, the researchers couldn’t tell if the one in seven exhibited an increase or a decline.

The executive director of the National Coalition of STD Directors and a social worker, David Harvey, said that irrespective of the absence of a comparison, these figures suggest that opioids must be contributing to the lesser explored impact on the lives of adolescents. For example, opioid use could be contributing towards the transmission of sexually transmitted diseases (STDs) in this age group.

Harvey pointed out that in 2007, at least 62 percent of teens reported having used condoms the last time they had an intercourse compared to 54 percent of teens in 2017. This decline along with the use of prescription drugs signals towards a teenager’s susceptibility to STDs like HIV and Syphilis. As many as 39 percent of students had sex in 2017 compared to 48 percent in 2007.

There was also a decline in the percentage of students encountering sexual dating violence from 10 percent in 2013 to 7 percent in 2017. This, along with a decline in the intake of alcohol and drugs, represented the wiser choices made by the students. The experts suggested that family support, especially the parental attention can make a lot of difference in an adolescent’s life. Further, an increased access to mental health and substance abuse resources can also make a lot of difference. Schools can contribute by offering coping skills and bystander intervention training.

Among the LGBT teens, there was increased incidence of risky behavior as their sense of physical and emotional well-being is threatened. In comparison to 2015, a higher number of gay, lesbian and bisexual teenagers reported having been raped. They also reported having missed school because of their concerns regarding their own safety.

Things Aren’t Always As They Seem

As a young child I enjoyed dressing up in my moms high-heeled shoes, jewelry and any piece of fancy clothing that I could get my hands on. It was like being transported to a magical land of make-believe. I would spend hours pretending to be a fairy princess or a celebrity! I would spend hours reading about and dreaming about love everlasting, moonlight kisses and magical romances. Again, this was enjoyable, but there would yet again come a time when I had to return to reality and embrace my life for what is was. Sometimes good, sometimes bad, but most of all tangible and real!

Still today as a wife, mother and mental health clinician I still find myself getting lost from time to time in a great day-dream. The key word however is dream and the reality that it is just that a series of thoughts, images and sensations that are occurring in my mind, not in my reality.

So often, however, it is hard for many to accept the everyday realities of life. This leads them to hide behind their titles, careers, money, marital status and so on. Many of us on the outside looking in buy into the mask, engage in the superficial conversation and walk away with the belief that because everything looks good it is good. This is so far from the truth!

With the recent tragic deaths of Fashion Designer, Kate Spade and Celebrity Chef and TV Personality, Anthony Bourdain, it has become all to clear that we must pause as a society and take the time to look behind the façade that so many put forth. We must begin today to ask the uncomfortable questions of those we love and often of those we have chance encounters with. Questions like:
• How are you doing spiritually?
• How is your marriage really going?
• How are you doing with your finances?
• How are you doing with your purity? In both thought and action.
• How are your private thoughts about yourself?
• What do you need most right now?

We can no longer accept the standard polite “I am good” or “things are fine” responses. If we claim to love someone we must be OK with maintaining eye contact during those few moments of awkward silence that often come when we ask the tough stuff.

We must begin to look past materialism and titles when we feel in our gut that something is just not right. Feelings of hopelessness are real. People are hurting. People are dying because of the stigma associated with Mental Illness. We no longer have the luxury of keeping it safely tucked away in the shadows only to be discussed in secret discreet whispers during family gatherings.

According to a 2016 Center for Disease Control Report, suicide claimed the lives of nearly 45,000 people and was listed as the tenth leading cause of death overall in the United States. However, suicide was the second leading cause of death among individuals between the ages of 10 and 34 and the fourth leading cause of death among individuals between the ages of 35 and 54. In 2016 there were twice as many suicides (44,965) in the United States as there were homicides (19, 362).

When you feel something is not right 9 times out of 10 something is not right. Below are a few warning signs that someone may be having suicidal thoughts:

• Unusual Focus on death – talking openly, dwelling on the topic or researching ways to kill themselves

• Making plans for death – updating their will, giving stuff away, saying goodbye, writing a suicide note

• Becoming withdrawn – avoiding close friends and family, losing interest in activities & social events, becoming isolated

• Showing signs of despair – talking openly about unbearable pain or feeling like a burden to others

• Changes in mood or sleep patterns – depressed, anxious, sad, or angry. They may also be very irritable, moody, or aggressive. They can also suddenly become calm once they have decided to go through with it. They may also sleep a lot more or a lot less

Proverbs 20:5 says “The purpose in a man’s heart is like deep water, but a man of understanding will draw it out”. We must start today to be that man or woman who will take the time draw out what is really going on within a person.
